Schedule a Visit
When you click the “Online Scheduling” button below it will open a new webpage with Elation Health.
- Use In the first box to select your preferred clinician’s name.
Scroll down in the drop-down menu to see all clinicians. - Scroll down on the main page to see all available
appointment options for your chosen clinician. - Once you have selected an appointment, it will ask for your information.
Please use your legal first and last name.

Please be aware that response times may vary depending on call volume and time of day. We aim to respond quickly but online scheduling is by far your quickest option.
